Microschool Enrollment Form Template

A complete enrollment form covering everything you need to collect before enrolling a student. Free to copy, adapt, and use.

What should a microschool enrollment form include?

A good enrollment form does two things: collects the information you need to safely and effectively serve each student, and creates a formal record of the family's agreement to your school's policies.

At minimum, every microschool enrollment form should cover student and family information, emergency contacts, health and allergy details, and a signature acknowledging your tuition and community policies. The template below covers all of this in eight sections.
